A. Application Requirements

  1. All applicants must not be older than 25 years old.
  2. For transferee applicants, slots are open for first year level only. Crediting of subjects taken shall be upon the college’s discretion.
  3. Secure and fill out an application payment slip at the Guard House.
  4. Pay the application fee of three hundred pesos (Php 300.00) and secure an application form at the Cashier’s Office. Application form can also be downloaded at this website: www.cghcnla.edu.ph
  5. Fill out the application form and submit it to the Guidance Office with the following:

a.    Three (3) pieces 2x2 picture. (1 piece for the application form; 1 piece for the test permit; 1 piece for the student information sheet)

b.    One (1) piece Long White Folder

c.    One (1) piece Long Brown Envelope

d.    Official receipt of payment for the entrance examination

Additional Requirements for Foreign Nationals:

a.    Original and photocopy of study permit issued by the Bureau of Immigration (for non-resident aliens only)

b.    Photocopy of Alien Certificate of Registration (ACR) I-Card.

c.    Original and photocopy of International English Language Test System (IELTS) Certificate

6. Secure examination permit and come back on the testing date.

B. Entrance Examination

  1. Take the entrance examination on the scheduled date. Examinees are requested to arrive 30 minutes earlier than the scheduled time. Late comers will be rescheduled. Proceed to the Guidance Office for rescheduling.
  2. Do not forget to bring (2) pencils and the ENTRANCE EXAMINATION PERMIT. No permit, no examination.
  3. The result of the entrance examination will be posted on specific dates as scheduled/announced.
  4. Transferees and second coursers from the Allied Health Science field will be required to take a diagnostic examination in Anatomy and Physiology.

C. Interview Screening

  1. Qualified examinees will be scheduled for Interview Screening.
  2. The following requirements are needed during the interview:                       

    For High School Graduates

    • Photocopy of NSO-authenticated Birth Certificate
    • Certificate from the school that the applicant belongs to the graduating batch OR certificate of good moral character if graduated
    For Transferees and Second Coursers:
    • Photocopy of NSO-authenticated Birth Certificate
    • Certified True Copy of Grades/Scholastic Records/Transcript of Records
    • Certificate of Good Moral Character
  3. Final list of Qualified Applicants for Admission will be posted on specific dates as scheduled/announced.

D. Reservation

  1. Applicants who passed the entrance examination and interview screening are required to pay a non-refundable reservation fee of Php 2,000.00 and medical examination fee of Php 500.00 at the Cashier’s Office.
  2. The reservation fee will be deducted from the tuition fee upon enrollment.
  3. Successful applicants who failed to pay the reservation fee on the dates given will forfeit their admission to the college and the slot will be given to other applicants.

E. Medical Examination

  1. Secure X-Ray and Dental Examination Schedule and Permit at the Cashier’s Office.
  2. Proceed to the X-ray Department (2nd floor) and Dental Clinic (Room 119-B) of Chinese General Hospital and Medical Center (CGHMC) as scheduled. Chest X-ray and dental examination must be completed prior to the scheduled day of physical examination.
  3. Present the results of Chest X-ray and Dental Examination to the College Physician at the College Clinic (Ground floor, Student Activity Center, CGHCNLA) on the scheduled day of physical examination.

F. Submission of Requirements for Enrollment

   Submit the following requirements at the Guidance Office after completion of medical examination:

For High School Graduates:

  •  High School Report Card (Original Copy)
  • Certificate of Good Moral Character (Original Copy)
  • NSO-Authenticated Birth Certificate (Original Copy)
  • Medical Examination Clearance issued by the College Physician

For Transferees and Second Coursers:

  • Course Description of all subjects taken
  • Certificate of Transfer Credential
  • Certificate of Good Moral Character (Original Copy)
  • NSO-Authenticated Birth Certificate (Original Copy)
  • Medical Examination Clearance issued by the College Physician

G. Enrollment

  1. Admission Slip shall be issued upon submission of complete requirements for enrollment.
  2. Date and time of enrollment is indicated in the admission slip. Please come on the specified date and time. Enrollees who failed to come on the specified date and time will be accommodated during the schedule for late enrollment.
  3. Read the reminders for enrollment at the back of the admission slip.
  4. Do not lose the admission slip. In case of loss of admission slip, secure a second copy from the Guidance Office.
  5. Foreign students will be required to pay non-citizen fee per semester/summer in the amount of US$ 250.00 for resident aliens and US$ 500.00 for non-resident aliens.
